Goal

Rewrite SKILL.md to a thin next_action dispatcher, remove the plan-creation phase, and relocate the meta-conventions.

Implementation Sketch

Collapse the SKILL.md body to a thin dispatcher that consumes the runtime’s next_action (no orchestration prose). DELETE the plan-creation phase entirely. Move the meta-conventions (reviewer-set / call-out-format / close-out / remediation conventions + the Findings-Disposition by-context table) out of the rule file into the skill/runtime. Keep SSOT-25 in impl-hygiene.md.
